SOME QUESTIONS ON FEMALE TO MALE - Barbara (for now), USA

I am 16 years old and I have always felt like a boy. Right now I am having a relationship with a girl who has no idea that I'm really a girl too.

I feel I am suppossed to be a boy because I interact much better with people as one. I am seriously considering a sex change and I wondered what kind of information there is on it.

Some questions I have are: Can a FTM have a fully functioning penis?

Can scars be hidden well?

How does your record change for the government?

People can't find out easily that you used to be a girl can they?

Your feelings are not uncommon, nor is the relationship you are in. That you interact better as a boy could indicate true transsexualism. However, you will require a full psychosexual and medical evaluation to determine that. For information on FTM sex change, there are lots of websites and support groups. Try searching in one of the major Search Engines. Details are outside the scope of this column. Now, the questions you asked...

  1. A fully functioning penis is possible if an inflatable prosthesis is inserted into the neophallus. This is a tricky operation, and one that must be performed only by experienced experts.

  2. Generally, scars in the genital region get hidden pretty well. But sometimes the donor areas, particularly if there's a radial artery free forearm flap taken, scar quite badly. Other determinants of scarring are surgical technique, surgical complications e.g. infections or fistulae, skin color etc.. Dark-skinned patients usually scar more than light-skinned ones.

  3. Government record changes depend on the law of the country. It varies. A few countries like the Netherlands and Sweden have very liberal policies for transsexuals. By contrast, a conservative theocracy can make a transsexual’s life miserable.

  4. Can people find out if you used to be a girl? Well, it depends on how well the reassignment job's been done, how motivated you are, and who's judging you. But if all three factors are favorable, it's sometimes nearly impossible to tell. Some conversions are truly unbelievable.

I am extremely interested in any new advances in phallaplasty. I am a 42 yr old FtoM transsexual although I despise the term. I have undergone 50 surgeries at the hand of a money hungry surgeon that has left me with 1 testicle implant properly positioned and one that is lost somewhere and a penis that is 1 and a half inches long, the only good thing is that the urinary hook-up worked. If you are looking for a willing patient I am your man.

I am a Transvestite. I've had a friend, who is a doctor of gynaecology. Two years I got hormones. But then he has to take a job in a foreign country so that I can't get other hormone-products. I don't want to go to another doctor. Is it possible to buy hormone products for breast development?

What country are you in and what drug were you taking earlier? If I have this information, I might be able to suggest something, but it's not a good idea to take these drugs without medical supervision.


INTERSEX DISORDER - P.S.M, Bombay, India

I am 17 years old and have been brought up as a girl. Indeed, I think I am one and feel like one. Over the last few years however, I have developed a beard, my voice has turned gruff and I have hypertrophy of the clitoris. There is no breast development and I have not started menstruating like my other friends. I am devastated. My family and friends treat me like I am some kind of curse. What is my problem? Is it curable?

You seem to be having an intersex disorder. This is a complex group of conditions where gender differentiation and delineation are abnormal leading to the development after puberty of girls becoming masculine and boys becoming effeminate. In developing countries these conditions are often detected quite late. You will need a detailed physical examination and some laboratory tests before a final diagnosis can be made and treatment is suggested.
