Sex is man's second strongest instinct. An instinct that is second only to the survival instinct. This means that if a man's life is not immediately imperilled, the next thing he will automatically think of is sex.

Before the advent of andrology, it was both thought and taught that impotence was psychological in origin. Epoch-making andrologic research in the past decade has shattered this myth and today it is known that in 80-90% of such cases, there is a physical (organic) cause rather than a purely psychological (functional) one.

Get your serum FSH, LH and Testosterone levels estimated and send me the values along with the lab's normal range alongside. Also, is your semen fructose-positive ?

NO EJACULATION ON ORGASMS - Linda, USA |
 |
I've been with my boyfriend for over a year now. He is 29 and a diabetic and has been for 15 years. Before we were together he began having difficulties with his erections, and he saw a Urologist for this. He didn't mention to the doctor that when he orgasms he doesn't ejaculate. He had this problem before he had troubles with his erections. Does this mean we may never have any children? If not is there a treatment?

Dr. Sudhakar Krishnamurti replies... |

There are many possibilities here:
- Diabetogenic erectile dysfunction (very common)
- Diabetogenic retrograde ejaculation (semen goes back into the urinary bladder)- also common
- Other cause of anejaculation. Check out our ejaculatory disturbances page on www.andrology.com/ejaculatorydisorders.htm
I'll need more details.

IS IT NORMAL? - Baskar, Karnataka, India |
 |
I'm 18, and I'm worried that I might be impotent. Everything I hear or read on other pages just doesn't match up with my problem. I've never had sexual intercorse, but my main concern is the curve of my erection... it points down, not up. While I can maintain this kind of erection, is it normal? Can my erection be "fixed" surgically... is it necessary?

Minor curvatures may not require correction. But major bends or those that preclude intromission (penetration) or those that cause psychosexual dysfunction will. Read the last paragraph in the section Curves, Bends and Peyronie's disease.

OLIGOSPERMIA - Marc, USA |
 |
I was wondering if you could tell me your views, on what would cause a low sperm count. My wife and I have been trying to make a baby for atleast 12 years, we have been married as long, please help.
|
Dr. Sudhakar Krishnamurti replies... |
 |
The causes of oligospermia are listed in detail on The Male Factor in Infertility. You will need a clinical examination and a few tests to determine what the cause might be in your case. Also, your wife might need a check-up. It appears that all you have done over 12 years is a semen examination. That's not enough.

MALE INFERTILITY - Arvind, Lucknow, India |
 |
I would like to know if a man with low sperm count of 20 million of which only 10% are actively motile can have a child by any assisted fertility methods available today. If so could you please let me know what are the methods and what will be duration of treatment. I am reaching 40 and my wife 38.
|
Dr. Sudhakar Krishnamurti replies... |
 |
Yes, of course. First, it has to be determined what the cause of the low sperm count is and whether it is curable. If it is, then we tackle that first. For instance, correctable causes of low sperm counts, e.g. varicoceles, respond very well to a microsurgical varicocelectomy operation performed at a center of excellence. If the low count is 'idiopathic' and does not respond to medication, then assisted reproduction techniques (ART) might help.
