- Before filling this form, please check out AGASSI's aims and objectives on previous page first.
- Please submit both, the online version as well as the printable version of the AGASSI membership form as part of your application process. The printable version should accompany the demand draft or printout of credit card payment confirmation or printout of EFT confirmation, by snail mail. This is necessary to facilitate record-keeping at our end. Thank you.

Such information will also help in sourcing key resource persons for different AGASSI leadership responsibilities and sectional activities. It will also be handy in identifying speakers/participants for scientific meetings/ conferences.
